Standard

The center implements a comprehensive, documented Quality Assurance (QA) system covering key aspects of its training operations (e.g., design, delivery, assessment, administration, resource management).

Rationale

A systematic QA approach ensures consistency in training quality, identifies areas for improvement, maintains standards, and builds stakeholder confidence.

Evidence/Indicators

  • QA policy and procedures manual/documentation.
  • Documented QA processes (e.g., internal audits, checklists, review cycles - PDCA).
  • Schedule and reports from internal QA audits/reviews.
  • Records of corrective and preventive actions (CAPAs) taken based on QA findings.
  • Designated staff roles/responsibilities for QA.
  • Evidence of QA system review and updates.

Metrics

  • % of key training operational processes covered by the documented QA system.
  • Internal QA audit schedule adherence rate (%).
  • Average time to close QA non-conformances or implement corrective actions.
  • Reduction rate (%) in recurring issues identified through QA audits.
  • Stakeholder (staff/trainer) satisfaction score with the QA system's effectiveness and usability.

Performance Levels

  • Non-Compliant (No System): No formal QA system or processes in place. Quality checks are absent or entirely ad-hoc.
  • Developing (Initial QA): Some basic quality checks exist but are inconsistent, undocumented, or cover very limited areas. Lacks a systematic approach.
  • Meets Standard (Developing QA): A documented QA system exists covering several key areas, but implementation may be inconsistent, or it lacks robust review/improvement mechanisms. Standardized checks applied somewhat inconsistently.
  • Exceeds Standard (Advanced/Fully Operational QA): A mature, comprehensive, documented, and consistently implemented QA system covers all critical operations. Includes regular reviews, data-driven improvements (CAPAs), clear roles, and continuous enhancement of the system itself.
